php相应问题

今天显得没事做了一个，然后就来问题了：只是做了一个填表然后上传保存文件，不知道哪错了。
附件已经上传

这个文件夹是存在的啊

我用的IIS启动 http://192.168.0.148/
然后upload之后就出现那个错误
